CT and MRI scans are two different types of imaging. We may use one of them to look closely at your sinuses or skull base. They're not needed for every patient. We order them only when a closer look is needed than an exam alone can give.

What is a CT scan?

A CT (computed tomography) scan uses X-rays to build a detailed picture of your sinuses and the bones around them. It's the standard way to see the shape of your sinuses, how much inflammation is there, and any blockages. This is usually the main imaging test used to plan sinus surgery.

What to expect: a CT scan is quick, usually just a few minutes. A routine sinus CT doesn't need any needles or injections.

What is an MRI?

An MRI (magnetic resonance imaging) scan uses a magnetic field, not X-rays, to show soft tissue in detail. It's especially useful for looking at the brain, nerves, and soft tissue near the skull base. For example, we may use it to check a CSF leak or a tumor that may reach beyond the sinuses.

What to expect: an MRI takes longer than a CT, usually 30–45 minutes, and means lying still inside a scanner. Some MRI scans use an injected dye to show certain tissues more clearly.

When is MRI useful?

MRI is not needed for every sinus problem. It is usually reserved for situations where more detail is needed about the soft tissues around the sinuses and skull base.

MRI may be recommended when there is:

  • A suspected nose, sinus or skull base tumor.
  • Concern that disease may involve the eye socket, brain or nearby nerves.
  • A suspected CSF leak or skull base defect.
  • Unusual or complicated sinus disease where CT does not provide enough information.
  • A need to better distinguish inflammation, fluid and soft-tissue abnormalities.
  • Previous surgery or complex anatomy where additional detail may help with planning.

CT and MRI often provide different information and may be used together in more complex cases.

Why it matters

Choosing the right imaging test, or knowing that none is needed yet, helps us reach an accurate diagnosis without extra tests. It also helps us plan treatment around what's actually happening in your case.
